We are concerned about your dental care and want to ensure you that it is performed in the most responsible manner. In order to assist you with the investment in your dental health, we have outlined our payment policy.

At South Bay Periodontics, we have several payment options available to ensure that you get the care that you need:

• Cash, check or  money orders

• Major Credit Cards

• Extended payment plan

• Insurance

Payment is due at time of service.

Dental Insurance Information

We submit claims to all dental insurance plans.  Insurance coverage varies.  Information outlining your individual benefits, such as whether your plan provides full or partial coverage for services, requires deductibles, or has annual maximum limits is available directly from your insurance company. Our helpful staff will submit any necessary forms for the services we perform to your insurance company as well as a pre-estimate when requested, in an effort to help you assess what you can reasonably expect to be covered.

After your initial visit with our periodontists you will have the chance to sit down with our financial coordinator and go over your insurance information as well as what we expect them to cover for your proposed treatment. We will gladly file a claim on your behalf. We will ask that you pay your estimated portion at the time of service. Upon receipt of an insurance payment, any balance due will be billed to you. If you have deposited an excessive co-payment, it will be refunded to you.
